What Is Intraoperative Monitoring (IOM)?
4 min readIntraoperative monitoring (IOM), also called intraoperative neurophysiological monitoring (IONM), tracks the function of the brain, spinal cord, and peripheral nerves in real time while a patient is under anesthesia. The goal is to alert the surgical team when neural pathways are at risk so they can adjust technique before lasting injury occurs.
Common modalities include somatosensory and motor evoked potentials, free-run and stimulated EMG, and EEG-based anesthesia monitoring, tailored to the procedure, from spine and neurosurgery to vascular and orthopedic cases where nearby nerves must stay intact.
Because IOM sits at the intersection of surgery, neurology, and anesthesia, facilities need a system with clear displays, reliable signal acquisition in an electrically noisy OR, and staff who are trained to interpret changes quickly during the case.
